context-refresh

Systematically rebuild mental model of any repository when starting fresh sessions. Uses multi-source semantic understanding (session history via rg + git commits + file changes) for complete context. Eliminates "cold start" problem in <3500 tokens. v3.1 uses proven chat-history-search patterns.
