Use qmd to index local files and search them.

- Add collection:

Notes
- Embeddings/rerank use Ollama at
OLLAMA_URL(defaulthttp://localhost:11434). - Index lives under
~/.cache/qmdby default. - MCP mode:
qmd mcp.
